All-on-4 vs All-on-6 Implants: Key Differences

All-on-4 and All-on-6 are both full-arch implant solutions that replace an entire jaw of teeth with a fixed, non-removable prosthetic bridge supported by titanium implants. Both treatments are transformative for patients with missing teeth or failing dentition — but there are important differences between the two that determine which is the better choice for each individual patient. This guide explains those differences clearly.

The Core Difference: Number of Implants

The fundamental difference is in the name: All-on-4 uses four implants per arch; All-on-6 uses six implants. In both cases, the implants support a full-arch fixed prosthetic bridge of 10–14 artificial teeth. The additional two implants in All-on-6 are typically placed in the molar region, improving posterior support and load distribution across the arch.

Bone Coverage and the All-on-4 Design Philosophy

All-on-4 was specifically designed by Nobel Biocare to work with the existing bone volume of the jaw by angling two of the four implants at up to 45°. This angling allows the implants to be placed in the anterior (front) portion of the jaw where bone is typically denser and more plentiful — avoiding the need for bone grafting in many patients. This is a key reason why All-on-4 can often be performed in a single surgical session without preliminary bone augmentation.

All-on-6 uses six straight implants, with the additional two placed further back in the jaw where there may be less bone. These posterior implants may sometimes require bone grafting, though this is less common than in traditional individual implant cases.

Who Is the Best Candidate for All-on-4?

  • Patients with moderate to significant bone loss in the posterior jaw
  • Patients who wish to avoid bone grafting surgery
  • Patients looking for the most cost-effective full-arch solution
  • Patients needing immediate teeth (All-on-4 strongly supports same-day loading)

Who Is the Best Candidate for All-on-6?

  • Patients with good bone density and sufficient posterior bone volume
  • Patients who want the maximum possible stability and load capacity
  • Patients with heavier bite forces or a history of bruxism (teeth grinding)
  • Patients seeking the most durable, long-lasting fixed arch solution

Which Is Better?

Neither is universally superior. All-on-4 is an excellent solution for the right patient and has decades of clinical evidence behind it. All-on-6 provides marginally better load distribution and is preferred for patients with the bone anatomy to accommodate six implants. The right choice is determined by your CBCT scan, bone anatomy, bite forces, and personal priorities.

At TakiDent, our oral surgeons assess every full-arch patient with a 3D CBCT scan and provide a clear, evidence-based recommendation. We never recommend the more expensive option unless it is genuinely the better clinical choice for the patient.
